Use FOR NEW COURSES, COURSE REVISIONS, CANCELLATIONS, GUR, FYE AND OTHERS. When completed, ATTACH to the E-SIGN FORMS for routing. The ATTACH button is located at the bottom of the E-SIGN FORMLink to E-SIGN FORMS for automatic routing (click on Academic Coordinating Commission on right side of page)

DIRECTIONS FOR COMPLETION:
The way the system works is that you must save the WORD FORM (a Printable document) to your hard drive and then attach it, along with the syllabus, to the E-SIGN FORM routing page. One way to think of the E-SIGN FORM is as something done on the Web. Western has many many of these E-SIGN FORMS, and the ones you want are located on an ACC link on the E-SIGN-FORM page. We also provided those links below.
In this particular case faculty must fill out a true E-SIGN FORM (the routing page) and then attach the WORD FORM ord documents (from a template to be found on the Web page indicated below), as needed. We are using the term E-SIGN FORM, for a form filled out on the Web, and a WORD FORM as a form found on the Web, but submitted as an attachment from one's hard-drive.

WORD FORMS and E-SIGN FORMS are available since Winter Quarter 2008 ONLINE. Dear Faculty: FYI There are Word Forms and E-Sign Forms (A WORD FORM is created when you save the available form into your own folder, fill it in, save it again, then send it as an email attachment to the required approver. These attachments are made to the routable E-Sign Forms) Please follow routing directions on the E-Sign forms. Thank you. |
