You will need to fill out a "2008 Enrollment/Change Form", and the "Declaration of Marriage/Qualified Domestic Partnership". Coverage will begin on the first of the month following the marriage date. You will have 60 days from the marriage date to enroll your spouse. If you do not enroll within the 60 days you will have to wait until the next open enrollment period. You have 60 days from the end of eligibility to remove your spouse/QDP. If you do not notify us your spouse/QDP may not be eligible to continue their benefits through COBRA.
