Frequently Asked Questions

I have recently married, and would like to add my spouse to my benefits. How do I do this?

You will need to fill out a "2009 Enrollment/Change Form", and the "Spouse or Qualified Domestic Partner Certification". Coverage will begin on the first of the month following the marriage date. You will have 60 days from the marriage date to enroll your spouse. If you do not enroll within the 60 days you will have to wait until the next open enrollment period. At this time you may want to also change your beneficiaries, and/or life insurance.

* If you are increasing any life insurance amounts other than Part E, you will also need to fill out the Life Insurance Evidence of Insurability Form.

** If you are a WWURP member or have a VIP with Fidelity, TIAA-CREF, or Vanguard, please contact your investment provider and request their beneficiary forms.

I had a baby. How do I add them to my benefits?

You will need to fill out a "2009 Enrollment/Change Form". Coverage will begin on the day the child was born. You will have 60 days from the birth to enroll your child. If you do not enroll within the 60 days you will have to wait until the next open enrollment period. At this time you may also want to change your beneficiaries, and/or life insurance.

I have adopted a child. How do I add them to my benefits?

You will need to fill out a "2009 Enrollment/Change Form", and the "Extended (Legal) Dependents Form". Coverage will begin on the date legal obligation is assumed for support in anticipation of the adoption. You will have 60 days to enroll the child. If you do not enroll within the 60 days you will have to wait until the next open enrollment period. At this time you may also want to change your beneficiaries, and/or life insurance.

I have met the criteria for a Qualified Domestic partnership. What forms do I need to add them to my benefits?

You will need to fill out a "2009 Enrollment/Change Form", and the "Spouse or Qualified Domestic Partner Certification". Coverage will begin the first day of the month following eligibility. You will have 60 days from initial eligibility to enroll your partner. If you do not enrolling within the 60 days you will have to wait until the next open enrollment period. At this time you may want to change your beneficiaries, and/or life insurance.

I am getting divorced, or my Qualified Domestic Partnership has ended. How do I remove them from my benefits?

You will need to fill out a "2009 Enrollment/Change Form". Their coverage will end on the last day of the month of the divorce, or partnership ending. You have 60 days from the end of eligibility to remove your spouse/QDP. If you do not notify us your spouse/QDP may not be eligible to continue their benefits through COBRA. At this time you may want to change your beneficiaries, and/or life insurance.
