As you prepare for retirement, you may find the following information useful.

Your state sponsored benefits will end effective on the last day of the month in which you retire. If you meet the definition (as defined by WAC 182-12-172) of an eligible retiree, you have the option to continue your medical, and/or dental insurance on a self-pay basis as a state retiree.

If you have participated in the optional life insurance plan while at Western Washington University, your life insurance (not accidental death and dismemberment insurance) continues for 60 days beyond your benefits end date. During that 60 days, you have the right to convert any amount of life insurance to an individual permanent whole life policy at the conversion rates for your age at that time. To apply for conversion, please complete and mail this form to ReliaStar Life Insurance Company: http://pebb.hca.wa.gov/forms/doc/50-540.pdf. In addition, there is a basic retiree life insurance plan available to you. For more information, please visit the Public Employees Benefit Board website at: http://pebb.hca.wa.gov/

For information that is extremely important to you in continuing your health insurance during your retirement years, please review the following documents carefully and complete the necessary forms for enrollment into the retiree health insurance plan. Send completed forms along with your first month’s health care premium directly to the Health Care Authority. You must enroll within 60 days of your retirement.
